What to Bring
Being prepared for your DOT physical can help your appointment go smoothly and avoid unnecessary delays. Please bring the following items to your appointment so we can complete your examination as efficiently as possible.
What to Bring to Your DOT Physical
Required Items
✔ Valid Driver's License or Government-Issued Photo ID
A valid photo ID is required to verify your identity before your DOT physical.
✔ List of Current Medications
Bring a complete list of all prescription medications, over-the-counter medications, and supplements you currently take, including dosages if possible.
✔ Glasses or Contact Lenses (If Worn)
If you normally wear corrective lenses while driving, please wear them to your appointment so your vision can be accurately evaluated.
✔ Hearing Aids (If Worn)
If you use hearing aids, be sure to bring and wear them during your examination.
Additional Documentation (If Applicaple)
Depending on your medical history, you may also need to bring additional documentation.
✔ CPAP Compliance Report
If you have been diagnosed with sleep apnea and use a CPAP machine, please bring your most recent compliance report.
✔ Diabetes Documentation
Drivers who use insulin or have diabetes may need additional documentation from their treating healthcare provider.
✔ Cardiac or Specialist Clearance
If you have a history of heart disease, recent surgery, seizures, or other significant medical conditions, you may need clearance or records from your specialist.
✔ Recent Medical Test Results
If you've recently completed stress tests, sleep studies, or other relevant testing, bringing those results may help prevent delays in certification.
Before Your Appointment
To help your visit go smoothly:
1. Stay well hydrated.
2. Continue taking your prescribed medications unless instructed otherwise by your healthcare provider.
3. Wear comfortable clothing for your examination.
4. Arrive a few minutes early to complete any necessary paperwork.
